Banksia Homeopathy is committed to protecting the privacy of your personal information and in protecting your privacy, I am bound by Australian Privacy Principles and the Commonwealth Privacy Act 1988.

Information I collect from you

I collect personal information including your name, date of birth, email address, mobile phone number, address, health fund and your medical history. I collect, store, use and disclose personal information for these reasons: to provide you with services (homeopathy) that you have engaged me to provide; to reply to questions emailed by you; to maintain your patient records electronically; to store your patient of information for the purposes of case analysis; for internal record keeping and administrative purposes; to store encrypted reports; to provide invoices and payment options; to maintain contact with you; for any purpose required or permitted by law.

Disclosing your personal information

Your health records contain sensitive information and I treat them confidentially. With your permission, I may disclose your personal information to another practitioner (supervisor or colleague, doctor, testing laboratory). Disclosing any of your personal information is only done with your consent. I will not sell any of your personal information.
